As a patient at LifeArts - Howard Chiropractic, PC, there are some forms that we need you to fill out and bring in to assist us with your care.

Prior to your first chiropractic appointment with Dr. Howard, please print off, fill out, and bring in the demographic form and the health history form.

Prior to your first nutritional assessment appointment with Dr. Howard, please print off, fill out, and bring in the assessment form and three (3) days worth of food records. Note: the nutritional forms are not needed for chiropractic care, only for nutritional assessments.
